Official Campaign Rules and Nomination Terms

NO PURCHASE OR PAYMENT IS NECESSARY TO SUBMIT A NOMINATION OR BE SELECTED. A PURCHASE OR PAYMENT WILL NOT INCREASE A NOMINATED HOUSEHOLD’S LIKELIHOOD OF SELECTION.

1. Campaign Overview

The Canfield Clean Air for a Cause Community Nomination Campaign, referred to in these Official Rules as the “Campaign,” is a community impact initiative sponsored by Canfield Heating, Cooling, Plumbing and Electrical, located at 411 E. Main Street, Lowell, Michigan 49331, referred to as the “Sponsor.”

The purpose of the Campaign is to identify and assist an eligible household within the Sponsor’s service area that may benefit from an indoor air quality improvement package.

Members of the community may nominate themselves or another eligible household by submitting a nomination describing the household’s circumstances and the potential impact of receiving the award.

This Campaign is not a random drawing, sweepstakes, raffle, or public voting competition. The recipient household will be determined by an internal review committee using the criteria described in these Official Rules.

2. Nomination Period

The Campaign begins on August 19, 2026, at 12:00 a.m. Eastern Time and ends on September 22, 2026, at 11:59 p.m. Eastern Time, referred to as the “Nomination Period.”

The Sponsor’s computer system is the official timekeeping device for online nominations. The Sponsor may also accept nominations at approved community events or through other authorized Campaign materials during the Nomination Period.

Nominations received before or after the Nomination Period may be disqualified.

3. Nominator Eligibility

A person submitting a nomination, referred to as the “Nominator,” must be a legal resident of the United States, be at least eighteen years old at the time of nomination, provide accurate and complete contact information, and agree to these Official Rules.

Employees, officers, directors, agents, and representatives of the Sponsor, Apex Service Partners, their affiliated companies, advertising or promotional agencies, and members of their immediate households may submit nominations but may not nominate themselves, members of their immediate families, or members of their households.

For purposes of these Official Rules, immediate family includes a spouse, parent, child, sibling, grandparent, grandchild, or the spouse of any such person.

4. Nominee and Household Eligibility

The household identified in a nomination, referred to as the “Nominee,” must meet all eligibility requirements before being selected.

  • At least one adult member of the household must be a legal resident of Michigan and at least eighteen years old.
  • The residence must be located within the Sponsor’s current service and installation area.
  • The residence must be the Nominee’s primary residence.
  • The Nominee must own the residence or provide written authorization from the property owner permitting equipment inspection and installation.
  • The residence must be suitable for the equipment and installation, as determined by the Sponsor.
  • The Nominee must agree to a home assessment or site inspection before final selection.
  • The Nominee must agree to the Campaign requirements, verification process, required media participation, and applicable releases.

A Nominee does not need to be an existing Canfield customer. The Sponsor may request reasonable documentation to confirm age, residency, homeownership, property-owner permission, installation eligibility, or other Campaign requirements.

Employees, officers, directors, agents, and representatives of the Sponsor, Apex Service Partners, their affiliated companies, advertising or promotional agencies, and members of their immediate families or households are not eligible to receive the award. The Campaign is void where prohibited or restricted by law.

5. How to Submit a Nomination

During the Nomination Period, an eligible person may access the Campaign nomination form through the Campaign landing page, a Campaign QR code, an approved community event, or another method authorized by the Sponsor.

Individuals may nominate themselves, their own household, or another eligible household.

There is no advantage to submitting the same household more than once. Duplicate nominations may be combined and considered as a single nomination.

The Sponsor may limit, remove, or disregard duplicate, automated, incomplete, misleading, fraudulent, or inappropriate submissions. All nominations become the property of the Sponsor and will not be returned.

The nomination form may request:

  • The Nominator’s first and last name
  • The Nominator’s email address and telephone number
  • The Nominee’s first and last name
  • The Nominee’s city or ZIP code
  • The Nominator’s relationship to the Nominee
  • A description of why the household should be considered
  • An explanation of how the indoor air quality package could benefit the household
  • Any other information reasonably required to administer the Campaign

6. Responsibilities of the Nominator

By submitting a nomination, the Nominator represents that the information provided is accurate to the best of the Nominator’s knowledge, the nomination is submitted in good faith, and the submission does not knowingly contain false, misleading, defamatory, threatening, or unlawful information.

The Nominator further represents that the Nominator has the right to submit any written materials, photographs, or other content included with the nomination and that the submission does not violate another person’s privacy, publicity, intellectual property, or other legal rights.

Nominators should not submit detailed private medical records, Social Security numbers, financial account information, or other highly sensitive personal information. A general description of respiratory concerns, allergies, household circumstances, financial hardship, or other relevant needs may be provided. Medical documentation is not required with the initial nomination.

A Nominator may submit a nomination without first informing the Nominee. However, the Nominee must provide consent and complete the Sponsor’s verification requirements before being selected.

7. Internal Review and Selection Process

All eligible nominations will be reviewed by an internal committee selected by the Sponsor.

The Campaign is based on evaluation and judgment. It is not a random drawing, sweepstakes, raffle, or public voting competition.

The Sponsor may contact Nominators or Nominees to request additional information, conduct interviews, verify eligibility, or schedule a site inspection.

The Sponsor may internally identify and evaluate multiple potential recipient households. Potential finalists or households under consideration will not be publicly announced.

Being contacted, interviewed, inspected, or considered by the Sponsor does not guarantee selection or receipt of the award.

The Sponsor retains sole and final authority over eligibility and the selection of the recipient household. The Sponsor’s decisions are final and binding, subject to applicable law.

The internal committee may consider:

  • The household’s demonstrated need
  • The anticipated health, comfort, or quality-of-life impact of the air quality improvements
  • Financial or personal circumstances described in the nomination
  • The presence of children, older adults, or household members experiencing respiratory or indoor air quality concerns
  • Community service, community involvement, or other circumstances supporting the nomination
  • The completeness, credibility, and overall strength of the nomination
  • The location of the residence within the Sponsor’s service area
  • The technical feasibility and safety of completing the installation
  • The Sponsor’s ability to provide the proposed equipment and installation within the Campaign timeline

8. Potential Recipient Consent and Verification

Before a household may be selected as the recipient, the adult homeowner or authorized resident may be required to confirm the household’s interest in participating, verify eligibility, consent to a preliminary home and equipment assessment, complete an eligibility affidavit and liability release, provide property-owner authorization if applicable, complete a publicity, photography, video, and media release, agree to participate in reasonable Campaign publicity and media activities, and provide other documents reasonably requested by the Sponsor.

The Sponsor will not publicly identify, photograph, record, or feature a household without first obtaining the necessary consent from an authorized adult member of the household.

If a potential recipient does not respond within the timeframe provided, declines participation or required media participation, fails to satisfy eligibility requirements, does not provide requested documents, cannot accommodate a site inspection, has a residence that is unsuitable for installation, or is otherwise unable or unwilling to comply with these Official Rules, the Sponsor may remove the household from consideration and select another potential recipient.

9. Selection and Notification of the Recipient Household

The Sponsor currently anticipates selecting the recipient household on or around October 15, 2026. This date is an estimate and may be changed by the Sponsor as reasonably necessary.

Potential finalists or other households under consideration will not be publicly identified or announced. Only the verified recipient household will be publicly announced.

The potential recipient will be contacted by telephone, email, or another method using the contact information provided through the nomination and verification process.

The potential recipient must respond within three business days after the Sponsor’s first notification attempt unless the Sponsor provides a different response period.

Failure to respond, satisfy eligibility requirements, participate in required publicity, complete required documents, or comply with these Official Rules may result in disqualification. The Sponsor may then select another household.

The recipient is not officially confirmed until the Sponsor has completed eligibility verification, the site assessment, all required documentation, and confirmation of the household’s agreement to participate in required Campaign publicity.

10. Community Impact Award

One eligible household is expected to receive a Canfield Clean Air Package with a total approximate retail value of up to $10,000, including approved equipment, materials, and standard professional installation.

The anticipated package may include one 403HR air purification system, one PMAC air cleaner, one HEPA filtration system, standard professional installation by Canfield, and other equipment, materials, labor, or installation components selected and approved by the Sponsor.

The advertised approximate retail value represents the estimated combined value of the equipment, materials, and standard installation included in the award. The actual value may vary based on the residence, equipment availability, installation requirements, and other factors.

The exact equipment, products, components, brands, models, specifications, and installation approach will be determined by the Sponsor following an evaluation of the residence.

The Sponsor may substitute equipment or a component with an item of comparable or greater value if the listed equipment is unavailable, incompatible with the home, discontinued, delayed, or otherwise impractical to install.

The award is nontransferable. No cash redemption, exchange, credit, or substitution may be requested by the recipient.

The Sponsor reserves the right, in its sole discretion, to provide an alternative indoor air quality package or installation solution if the advertised equipment cannot reasonably or safely be installed.

11. Installation Requirements and Limitations

Installation is subject to a home assessment and final approval by the Sponsor.

The residence must meet all technical, structural, mechanical, electrical, ventilation, access, safety, permitting, and code requirements necessary to complete the installation.

The award includes only the equipment and standard installation expressly approved by the Sponsor.

Repairs, replacements, modifications, remediation, code corrections, ductwork alterations, electrical upgrades, structural work, hazardous-material removal, or other additional work required to make the residence suitable for installation are not included unless expressly approved in writing by the Sponsor.

The recipient will not be required to purchase additional products or services as a condition of receiving the award. If additional work is necessary and is not included, the Sponsor and recipient must agree in writing on how to proceed before any chargeable work is performed.

If the installation cannot be safely or legally completed, the Sponsor may modify the equipment package, offer an alternative indoor air quality solution, select another eligible household, or cancel the proposed installation without further obligation.

Installation must be scheduled and completed within the timeframe communicated by the Sponsor. Failure to reasonably cooperate with scheduling or provide access to the residence may result in forfeiture.

The Sponsor is not responsible for delays caused by permitting, inspections, product availability, weather, labor availability, emergencies, supply chain disruptions, property conditions, acts of government, or other circumstances outside the Sponsor’s reasonable control.

The Sponsor is not responsible for property damage or installation complications caused by concealed, undisclosed, unsafe, defective, or preexisting conditions at the residence.

Installation requirements may include:

  • Suitable existing HVAC equipment and ductwork
  • Sufficient physical space and safe equipment access
  • Appropriate electrical capacity and ventilation
  • A structurally and mechanically sound installation area
  • Compliance with applicable building and safety codes
  • Permission from the property owner, condominium association, homeowners association, or other controlling authority, if applicable

12. Taxes and Financial Responsibility

The recipient is responsible for all federal, state, and local taxes associated with the acceptance and use of the award.

The fair market value of the equipment and installation may be treated as taxable income and reported as required by law.

As a condition of receiving the award, the recipient may be required to provide a completed IRS Form W-9 or other tax documentation. The recipient may receive an IRS Form 1099-MISC or another applicable tax form reflecting the reportable value of the award.

Failure to provide requested tax documentation within the timeframe established by the Sponsor may result in forfeiture. The recipient should consult an independent tax professional regarding the potential tax consequences of accepting the award.

13. Privacy

Information collected through the Campaign will be handled in accordance with the Sponsor’s Privacy Policy, available at https://www.canfieldph.com/privacy-policy/.

The Sponsor may use submitted information to administer the Campaign, verify eligibility, contact Nominators and Nominees, evaluate nominations, conduct site assessments, coordinate equipment and installation, meet tax, insurance, legal, or regulatory requirements, and obtain consent for authorized Campaign publicity.

Submitting a nomination does not automatically enroll the Nominator or Nominee in marketing communications unless separate, clear consent is provided.

The Sponsor may share information with its parent companies, affiliates, contractors, legal advisers, tax professionals, insurers, service providers, and other parties as reasonably necessary to administer the Campaign.

Nomination stories will not be publicly shared in personally identifiable form without appropriate consent from the Nominee or an authorized adult representative of the household.

14. Required Publicity and Media Participation

Participation in reasonable Campaign publicity and media activities is a condition of accepting the Community Impact Award.

Before the award is confirmed, the potential recipient must sign a written publicity, photography, video, and media release provided by the Sponsor.

Subject to that release and applicable law, the recipient agrees that the Sponsor may use the recipient’s and participating household members’ names, city of residence, likenesses, voices, statements, nomination story, photographs, video and audio recordings, installation footage, before-and-after images, and experience receiving and using the award.

The Sponsor may use these materials for advertising, public relations, news outreach, websites, social media, paid media, internal communications, community education, and other promotional purposes in any media now known or developed later, without additional compensation.

Required participation may include reasonable cooperation with photography and video recording before, during, or after installation; interviews with Canfield representatives; interviews or appearances involving local news or community media; a public recipient announcement; social media and website content; and follow-up content concerning the household’s experience with the installed equipment.

The Sponsor will make reasonable efforts to schedule media activities at mutually workable times and to respect the household’s dignity and privacy.

The recipient will not be required to publicly disclose confidential medical records, detailed financial records, Social Security numbers, or other highly sensitive information. A general description of the household’s circumstances, indoor air quality concerns, and expected benefit from the award may be included in Campaign publicity.

Information, images, or statements involving a minor may be used only with the written consent of the minor’s parent or legal guardian.

If the potential recipient declines to sign the required release or declines to participate in reasonable Campaign publicity, the household may be disqualified and the Sponsor may select another recipient.

15. Nomination Content License

By submitting original written materials, photographs, videos, or other content, the Nominator grants the Sponsor a nonexclusive, royalty-free license to review, copy, store, and use the materials for administering and evaluating the Campaign.

The Sponsor will not publicly use identifiable nomination content without obtaining any additional permissions or releases reasonably required. The Nominator retains ownership of original nomination content.

16. General Conditions

The Sponsor reserves the right to disqualify a Nominator, Nominee, or submission that violates these Official Rules; contains false, misleading, incomplete, or unverifiable information; infringes another person’s rights; attempts to manipulate or interfere with the review process; uses automated submission methods; harasses or threatens Campaign participants or representatives; damages or attempts to damage the Sponsor’s website, systems, or Campaign administration; or otherwise undermines the integrity or intended purpose of the Campaign.

The Sponsor reserves the right to cancel, suspend, extend, modify, or terminate the Campaign if fraud, technical failures, emergencies, legal requirements, equipment availability, safety concerns, or other circumstances impair the Campaign’s administration or intended purpose.

If the Campaign is modified or terminated, the Sponsor may select a recipient from eligible nominations received before the modification or termination, provided doing so is lawful, safe, practical, and consistent with the Campaign’s purpose.

The Sponsor’s failure to enforce any provision of these Official Rules does not constitute a waiver of that provision.

17. Release and Limitation of Liability

To the fullest extent permitted by law, each Nominator and participating Nominee agrees to release and hold harmless the Sponsor, Apex Service Partners, their affiliates, advertising and promotional agencies, contractors, equipment manufacturers, and their respective officers, directors, employees, representatives, and agents from claims arising from submission of a nomination; participation in the Campaign; selection or nonselection of a Nominee; acceptance, installation, possession, use, or misuse of the award; authorized use of nomination, photographic, video, or publicity materials; technical failures or interruptions; errors or omissions in Campaign materials; or acts or omissions outside the released parties’ reasonable control.

Nothing in these Official Rules is intended to release a party from liability that cannot legally be waived.

The Sponsor is not responsible for lost, late, incomplete, corrupted, damaged, misdirected, illegible, or unauthorized nominations or for telecommunications, internet, website, software, hardware, or system failures.

Except for warranties expressly provided in writing, the equipment is provided subject to applicable manufacturer warranties. Ongoing maintenance, filter replacement, repairs, utilities, and future service are the recipient’s responsibility unless otherwise stated in writing.

18. Disputes and Governing Law

The Campaign and these Official Rules are governed by the laws of the State of Michigan, without regard to conflict-of-law principles.

To the extent permitted by law, any dispute arising from the Campaign must be brought individually and not as part of a class action.

Any legal proceeding relating to the Campaign must be filed in a court of competent jurisdiction located in Michigan unless otherwise required by applicable law.

To the extent permitted by law, recovery is limited to actual out-of-pocket expenses directly associated with participation in the Campaign. Participants waive claims for punitive, incidental, special, indirect, or consequential damages and attorneys’ fees.

19. Severability

If any provision of these Official Rules is found to be invalid, unlawful, or unenforceable, that provision will be enforced to the maximum extent permitted, and the remaining provisions will continue in full force and effect.

20. Recipient Information

After the recipient has been verified and publicly announced, a request for the recipient’s name and city may be submitted by sending a self-addressed, stamped envelope by November 5, 2026, to: Canfield Clean Air for a Cause, Recipient Information Request, 411 E. Main Street, Lowell, Michigan 49331.

Information will be provided only as permitted by applicable law and the recipient’s executed publicity release.

22. Acceptance of Official Rules

By submitting a nomination or agreeing to participate as a Nominee, each participant acknowledges that they have read, understood, and agreed to the applicable provisions of these Official Rules.

The Sponsor’s interpretations and decisions concerning these Official Rules and the Campaign are final and binding, subject to applicable law.
